Home from Hospital

No one wants to stay in hospital longer than necessary – Most people wish to recover in the comfort of their own home, especially if they are medically fit to do so. However, getting home from a hospital stay is not always straightforward.

Prior to discharge an Occupational Therapist Assessment may be required to arrange essential equipment. Medications need to be organised, and sometimes a “Package of Care” must be in place to ensure safety and support at home. Without this planning, discharge can be delayed.

At Great Park Homecare our Rapid Response Service to our Domiciliary Support is designed to make the transition home from hospital smooth and stress free. We are available to deliver to you seven days a week from 8am to 10pm, working with you and your family to arrange the right support as quickly as possible.

An example of this was a call we received one New Year’s day at midday. We undertook an assessment at 3pm and delivered our first support visit at 7pm that evening. This is the level of responsiveness and dedication you can expect from Great Park Homecare

We’re so happy to help you

Home care is personalized support provided to individuals in their own homes, helping them with daily activities, medical needs, or companionship. It's ideal for seniors, people with disabilities, or those recovering from illness, allowing them to maintain independence, comfort, and dignity while receiving professional or non-medical assistance.

Home care may be right for you or your loved one if support is needed with daily tasks, mobility, or health management at home. It offers flexible, personalized care while promoting independence and comfort—ideal for seniors, individuals with disabilities, or those recovering from surgery or illness.

You should consider home care when daily tasks become difficult due to aging, illness, or disability. Signs include struggling with personal care, household chores, or medication management. Home care offers support while allowing individuals to remain safely and comfortably in their own homes.

A needs assessment is an evaluation carried out by a professional (often from social services) to determine the type and level of care or support a person requires. It considers physical, emotional, and social needs to create a personalized care plan, especially for elderly or disabled individuals.

Yes, you or your loved one may be eligible for benefits such as Attendance Allowance, Personal Independence Payment (PIP), or Carer’s Allowance. Eligibility depends on individual circumstances, including age, health conditions, and level of support needed. A local authority or benefits advisor can help assess what support is available.

The amount of help needed depends on the individual’s daily living challenges—such as mobility, personal care, meal preparation, or medical needs. A care needs assessment by a local authority or private provider can determine the right level of support, from occasional visits to full-time care, based on personal circumstances.

Home help is non-medical support provided at home to assist with daily tasks such as cleaning, cooking, shopping, and personal care. It helps individuals—especially the elderly or disabled—maintain independence and quality of life while remaining in their own homes.

**Home care** typically involves personal care tasks like bathing, dressing, medication support, and mobility assistance, often provided by trained carers. **Home help** focuses on practical support such as cleaning, cooking, shopping, and companionship. While both support independent living, home care includes more health-related assistance.

Alternatives to home care include assisted living facilities, nursing homes, adult day care centers, and respite care services. These options provide varying levels of support outside the home, offering social interaction, medical supervision, or temporary relief for family caregivers, depending on individual needs and preferences.

To make a house safe for home care, consider installing grab bars in bathrooms, improving lighting, removing trip hazards like loose rugs, adding non-slip flooring, widening doorways for accessibility, using stairlifts or ramps, and ensuring easy-to-reach switches and emergency alert systems are in place.

Client Testimonial

It is difficult knowing where to begin, and I am unused to writing reviews for anything! However, all my dealings with Great Park Homecare have always been excellent. They provide individualised care. I know my husband is getting the best he can from the carers and the company. These comments are set against experiences of at least three other providers during the last 11-years. Clearly, the last 12-months or so have been “The Best” for us.

Great Park Homecare has been a wonderful support for us. Their individualised approach ensures my husband receives excellent care every day.
